BH/DS Clinician II

Description:

The City of Virginia Beach Department of Human Services is seeking a qualified Clinician II for the DS Case Management program in the Developmental Services Division. The ideal candidate will have at least three (3) years of experience working with the ID/DD population and be certified as a QDDP with the VA Board of Licensing, including one (1) year of experience as an ID/DD Case Manager.

Duties:

  1. Perform complex assessments and clinical interventions (including consumers with dual diagnosis) in case management.
  2. Develop routine service plans with consumers, families, caretakers, or relevant parties, and coordinate and monitor consumer care and services.
  3. Lead the implementation of planned consumer services and strategize the development of needed resources and services.
  4. Provide timely case documentation in accordance with quality assurance, Medicaid, licensing standards, and professional ethics.
  5. Deliver direct case management services based on thorough assessments of consumer needs and preferences.
  6. Maintain a caseload of 30+ Medicaid and non-Medicaid consumers.
  7. Conduct initial intakes and routine assessments to evaluate consumer interests, strengths, and needs.
  8. Develop clear, individualized service plans tailored to consumer needs.
  9. Meet with clients as per the established schedule.
  10. Accurately enter consumer data into the electronic healthcare record system within deadlines.
  11. Ensure all documentation complies with standards set by the Continuous Quality Improvement division and Medicaid/licensure requirements.
  12. Respond effectively to crisis or emergency situations with supervisory support.

  • $1,000 awarded in the first paycheck.
  • $1,250 awarded after a six-month performance review with a "Meets Expectations" rating.
  • $1,750 awarded after one year of employment with a "Meets Expectations" rating.
