Are you looking for meaningful work with families of infants and toddlers with developmental delays or disabilities? The Infant Toddler Program in Twin Falls, Idaho is currently seeking a Service Coordinator with education and experience in case Management/service coordination. This position will include working within a multidisciplinary team to determine and implement services in the child's natural environment. Responsibilities also include intake, assessment, eligibility determination, development of Individualized Family Service Plans, resource development, collaboration with community partners, and ongoing service monitoring for children and families involved with Infant Toddler Program.
Travel is required in Twin Falls and surrounding areas and a state car is available for program related travel. In addition, you will work alongside some of the most caring and compassionate employees who strive to make a difference in people's lives every day.
The Idaho Department of Health & Welfare is a drug-free workplace.
