The Care Coordinator will provide outreach, navigation and coordination of care within the healthcare system for our patients with Diabetes, Hypertension, and those due for preventative services with a focus on those who are uncontrolled. The Care Coordinator is expected to exercise a high degree of initiative and judgement in coordinating patient care and follow up. Bilingual English/ Spanish preferred.$16.50 - $18.50 DOE. Employee's can receive up to $520 on the quarterly bonus. MINIMUM QUALIFICATIONS: - High school diploma or GED equivalent.
