Qualifications PT Licensed in the State of IdahoCurrent CPR certificationValid Driver's LicenseAuto InsuranceHome Health and Hospice experience preferredResponsibilities Provide physical therapy services to patients according to a written physician's plan of care.Assess and evaluate therapeutic, rehabilitative, and functional status, and participate in development of the total plan of care.Evaluate home environment.Assess muscle strength, mobility, gait, ROM, and potential for rehab.Direct physical therapy treatment.Instruct patients and families/caregivers in use and care of therapeutic appliances.Determine priority needs for physical therapy.Report to physician regarding patient's reaction to treatment or changes in condition.Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of therapy, as well as instructing them regarding the goals of the physical therapy program for the patient by participating in case conferences.May train patient in the use of a prosthetic device.Identify patient and family/caregiver needs for other home health services and refer as necessary.Prepare and submit clinical and progress summaries based on attainment of goals.Participate in discharge planning for patient.Provide physical therapy consultation to home families/caregivers when indicated.Provide in-service education programs for nursing organization personnel as needed.Participate in peer consultation process.Supervise Physical Therapy Assistants according to organization policy and state regulations.Adhere to organization's documentation and care procedures and standards of personal and professional conduct.When therapy is the only skilled service, instruct, supervise and evaluate home health aide care every two weeks.Location Primary: Twin Falls, IDSecondary: Burley, IDCompensation $43.00- $48.00 per hourEmployment Status Non-Exempt PRNEEO Statement Horizon Home Health and Hospice is an Equal Opportunity Employer.
