SUMMARY:Under the general supervision of the Chief Information Officer (CIO), the Quality Improvement Coordinator supports FHS clinics in improving quality measures, including but not limited to diabetes, hypertension, preventive screenings, and immunizations. This position provides reports to clinics, validates data related to Plan-Do-Study-Act (PDSA) cycles, and supports other quality improvement activities. The Quality Improvement Coordinator is expected to exercise initiative, sound judgment, and analytical skills while working closely with clinical staff and administration to improve performance across quality measures.Salary:$23.86 - $30.82 DOEMINIMUM QUALIFICATIONS AND ABILITIES REQUIRED:High school diploma or GED equivalentExcellent communication skillsStrong interpersonal skills and the ability to work effectively with patients, clinical staff, and specialistsMedical terminology experience preferredProficiency with Microsoft Office programsPossess good organizational and time management skillsMust be detail oriented and organizedAbility to exercise discretion and maintain patient privacyAbility to work independently and collaboratively as part of a teamValid Idaho Driver's LicenseAbility to pass a background checkDUTIES AND RESPONSIBILITIES:Runs reports to monitor quality measures at the provider, clinic, and organizational levelsCreates graphs, lists, charts, and other data summaries to support quality improvement effortsOrganizes and analyzes data to identify trends and opportunities for improvementValidates data to ensure accuracyDisseminates information in a clear, logical, efficient, and useful mannerCompletes all functions of the recall process including running the recall report, printing recalls, mailing recalls, updating the EHR, correcting addresses, etc.Participates in Patient-Centered Medical Home (PCMH) activitiesManages Patient Portal activities with clinicManages Patient Outreach campaignsActively involved with the QA/QI CommitteeParticipates in local and state quality improvement conferences, programs, projects and trainingsCommunicates effectively with providers, nurses, administration and othersFollows FHS policies and procedures when documenting in the EHRAssists with data collection, patient registries, documentation, and reportingCompletes the weekly Jean's Friday report and works with clinics that do not meet Jean's Friday goals to support improvementParticipates in ongoing trainings, conference calls, webinars, and professional development opportunitiesPerforms other duties as assignedOTHER RESPONSIBILITIES:SAFETY:Family Health Services enforces a safety culture whereby all employees have the responsibility for continuously developing and maintaining a safe working environment.
